Santa Clara Family Health Plan Scholarship

Opportunity Information
The Santa Clara County Family Health Plan Scholarship is $5,000 annually for full-time students, and $2,500 annually for part-time students.

If awarded, students are eligible for the scholarship for up to two years (4 semesters). The number of scholarships available will be determined by number of qualified applicants, with up to two scholarships being awarded. Priority will be given to full-time students.

Qualifications

  • Applicant must be a Santa Clara Family Health Plan Medi-Cal Member.
  • Applicant must be in a degree or certificate program in an approved area of study, including: Kinesiology, Medical Assisting, EMT, Dental Assisting, CNA, Community Health Worker, Nursing, and Public Health.
  • Applicant’s cumulative GPA must be a 2.50 minimum.
  • Applicant must currently be enrolled at least as a half-time student (i.e. enrolled in at least 6 units).

  4. By checking this box, you acknowledge that applicants awarded a scholarship will be required to attend an in-person award ceremony hosted on campus at San Jose City College. Attendance at the award ceremony will be mandatory for all scholarship recipients, unless the student has a class scheduled during the time of the ceremony or if the student has an extenuating circumstance. You also acknowledge that you will be required to submit a "Thank-You Letter" to the donor(s) providing the scholarship opportunity.
